Chapter 2. Patient Assessment Show
A comprehensive head-to-toe assessment is done on patient admission, at the beginning of each shift, and when it is determined to be necessary by the patient’s hemodynamic status and the context. The head-to-toe assessment includes all the body systems, and the findings will inform the health care professional on the patient’s overall condition. Any unusual findings should be followed up with a focused assessment specific to the affected body system. A physical examination involves collecting objective data using the techniques of inspection, palpation, percussion, and auscultation as appropriate (Wilson & Giddens, 2013). Checklist 17 outlines the steps to take. Checklist 17: Head-to-Toe Assessment
Which action would the nurse take when assessing the neck area of a patient quizlet? For which lymph nodes would the nurse check while assessing the patient's neck? Cervical lymph nodes are located in the neck region. Therefore the nurse would assess for cervical lymph node enlargement while assessing the patient's neck.
What does the examiner assess While examining the head and face of a patient?Face. Careful inspection of the face as a patient talks will reveal many factors. The examiner should look carefully for signs of trauma, previous facial surgery, eye swelling, or "bags under the eye," which might indicate fluid or allergic disorders. The quality of the skin can be quickly noted.
What does the nurse palpate while assessing the inguinal area of a patient? The large artery present in the inguinal area is known as the femoral artery, which is present near the inguinal folds and supplies blood to the lower limbs. Therefore the nurse palpates the femoral artery while examining the inguinal area.
Which technique would the nurse use to examine the patient's lymph nodes?Palpation: The nurse performs a complete breast examination using the finger tips to determine if any lumps are felt. The lymph nodes in the axillary areas are also palpated for any enlargement or swelling.
