The elderly often do not experience obvious SS that younger people have, such as diff urination, or freq urination. Instead they exhibit more vague SS that are similar to many other diseases or may be assumed to be due to the aging process. SS include: confusion, feelings of general discomfort, disorientation, fatigue, weakness, behavior changes, falling, and/or new acute incontinence. B/c incontinence is common in elderly, they may not be aware of the symptom of frequency.

S&S depend upon severity of injury:
- Acute blood loss: (shock) = hypotension, tachycardia, tachypnea, cool pale skin, altered LOC
Observe for hematuria, gross or microscopic
Flank or abdominal pain
Localized swelling, tenderness, ecchymosis in flank area
Diagnostic tests/monitor for:
- UA (hematuria)
- H&H (decreasing values = bleeding)
- CT, MRI, IVP with cystography, ultrasound; maybe renal arteriogram

- Treat the underlying cause!
- Prevention = frequent monitoring for early signs of AKI
- Assess for FVD vs FVE: VS, Strict I&O, Daily weights, - Monitor labs (Hgb, Hct, WBC, Creatinine, BUN, Creatinine clearance, K, Na, Ca, Phos, ABGs)
- Diet = Fluid restriction, Low K+, low Na, Low protein (kidneys cannot dispose of protein & it breaks down into nitrogenous waste)

a, b, c, d, e.

High-risk patients include those exposed
to nephrotoxic agents and advanced age (a), massive trauma (b), prolonged hypovolemia or hypotension (possibly b and c), obstetric complications (c), cardiac failure (d), preexisting chronic kidney disease, extensive burns, or sepsis. Patients with prostate cancer may have obstruction of the outflow tract, which increases risk of postrenal AKI (e).

What happens in oliguric phase of AKI?

Oliguric Phase: The most common initial clinical mani- festation of AKI is oliguria, defined as a reduction in urine output less than 400 mL/day. Oliguria is manifested with- in 1 to 7 days of kidney injury. This phase typically lasts 10 to 14 days but can last months in some cases.

Which nursing intervention is appropriate for a patient during the oliguric phase of an acute kidney injury AKI )?

The answer is C. The patient with AKI, especially in the oliguric stage of AKI, should eat a low-protein, low-potassium, and low-sodium diet. This is because the kidneys are unable to filter out waste products, excessive water, and maintain electrolyte balance.

What is oliguric acute kidney injury?

Oliguria is defined as a daily urine volume of less than 400 mL and has a worse prognosis. Anuria is defined as a urine output of less than 100 mL/day and, if abrupt in onset, suggests bilateral obstruction or catastrophic injury to both kidneys.

What does the term oliguria refers to quizlet?

Oliguria is defined as a urine output that is less than 1 mL/kg/h in infants, less than 0.5 mL/kg/h in children, and less than 400 mL daily in adults.
